Radio Buttons

I’ve built a form that rates teachers in multiple categories. Each category has three radio buttons Excellent, Satisfactory and Unsatisfactory.
I place five radio buttons on the form. Problem is that the forms sees all five radio buttons as one group. In other words, picking Excellent in the second radio button, clears it in the first. Only one pick is shown across all five buttons. I want one pick on each button.
