Broken on activation

Upon activating the plugin I received the following error with debug turned on:

Along with this error rendered in the page itself:
The plugin generated 191 characters of unexpected output during activation. If you notice “headers already sent” messages, problems with syndication feeds or other issues, try deactivating or removing this plugin.
It appears the two are actually different errors. I’m surprised no one else has seen either, but I suppose it’s possible there’s a plugin conflict.
Please advise on how else I can assist in tracking these down.
