After update to 1.0.69 my form doesn't render

Strange. After updating the form a second time it is loading fine.

The issue you are reporting seems to be a cache problem, the browser is trying to load the javascript files related with the previous version of the plugin. In this case, you can go to the settings page of the plugin, and untick the checkbox: “Activate Javascript Cache”, press the “Update” button, and refresh the webpage with the form.

and returns the homepage instead of the javascript code, however, if you visits any other URL with the same parameters, your website does not intercepts the URL, and returns the javascript code:

https://www.calcolocessionedelquinto.it/pensionati-inps.htm?cp_cff_resources=public&min=1&ver=1.0.69

Please, go to the setting page of the plugin, and tick again the option: “Activate Javascript Cache”, and press the “Update” button.
