What do you mean, “fields are a bit messy”? Can you clarify?

In regards to the non us country having to select a state, we will look into building in some sort of JavaScript.

Finally, the labels are all controlled on MailChimps side. If the fields aren’t translated you may want to look into localization settings in your mailchimp account.
