Form will not submit!

hey guys!
so i’m still a bit of a newbie when it comes to plugins. i’ve scoured the internet for a fix to this, but i can’t seem to figure out what i’m doing wrong. (why do i get the feeling it’s something really silly?)

upon hitting submit, the box grays out like it’s SUPPOSED to submit, but i don’t get a message explaining that NOR does my entries count go up.

troubleshooting i’ve done:
– disabling each element’s “required” option
– testing the plugin on multiple, recently-updated themes
– disabling all other plugins
