Contact Us Form Issue

Hi,

I could not figure out why I am seeing Name (Required) “gally192″ and Email (Required)”[email protected]” in the textbox while Subject and Message are showing blank. The site is not yet in the public. Here is the code:

I am more than happy to send you the screen shot of it but it does not have the ability for me to attach the image here.
