Followed upgrade steps but still receiving Warning message

Have followed the steps multiple times for doing the upgrade, including via FTP, but still receiving this message on the Donate page (also created a new test page which brings back the same error):

Warning: This form needs to be updated. Please update using the [seamless-donations] shortcode.

Is this one of the issues that comes up with upgrading from v3 or something else?
