Radio Buttons not showing up

    Did you update the source Google Form? Google has made a chance to Google Forms so within Google Docs if you create a new form or agree to migrate an existing form, your form will take on the new structure which is currently incompatible with the plugin.

    There are a couple posts on my site about it here and here.

    No if you didn’t migrate your form or create a new one, I would then look to see if you had Custom CSS which is not being loaded for some reason. Did anything else change on your site?
