How do I add a option for custom value in "Other" Checkbox in Forms?

  • Hello,

    I have added a form to my organizations website the options for the checkboxes are…

    Personal Use or Recreational Fisherman
    Subsistence Fisherman and/or Hunter
    Commercial Fisherman
    Crew Member
    Permit/IFQ Holder
    Shellfish Farmer
    Other

    I would like to have the option for “Other” to say Please Specify – and give the user an option to write something in. How do I do this?
