forgot password link reloads profile page

The user profile shortcode page needs to be unblocked if you are blocking pages. Otherwise the user can’t get to the password reset process.

I’m not sure why you are using all of those shortcodes. Maybe it’s not clear in the documentation, but the [wp-members page=user-profile] does by itself all of what you have there. Get rid of all of the extra shortcodes and just use the user profile page shortcode.
