Form Shortcode Display Issue

Hello, I am attempting to use the Give plugin on a client’s website, but while the form looks perfect in the “View Form” window, when I use the shortcode to put it into the page I’ve created, the text entry fields are whited out and not visible, you cannot enter the custom donation amount, or select/enter anything else. It’s like the entire form is being cut off on the full-width webpage and all I can see are the form names, text field titles, and a dollar sign.
