FormBuilder Plugin 1.0.5 and Auto Fill Problem

Problem: some browsers (not all, but Safari for one) auto fill my info (Name: admin, Email: [email protected]) in those two fields, Name and Email. Not only does this occur on my computer/browser, but on other users’ computers as well!
Can anyone help?
