Can't get New Relic to integrate with W3TC

I’ve installed New Relic on our dedicated server (CentOS with Nginx), and can access the data on their site, but can’t get it to work in W3TC. The API key verifies okay, but I can’t obtain an app id. I’m not even sure what “Application name” is referring to, and don’t see that adequately explained anywhere.
The message W3 gives me is:
New Relic is not running correctly. The plugin has detected the following issues:
PHP module is not enabled.
PHP agent is not enabled.
Application ID is not configured. Enter/Select application name.
License key could not be detected.However, the PHP module & agent must be enabled, since I’m getting server data from the New Relic site, right? The license key should be fine too, it was configured on the back end when I installed via SSH.
There really should be some better documentation provided by W3, I think, but it just refers to New Relic’s site, which is no help for the integration either.
