From Frontend User Form to Custom Design Blog Post

I’m using the WordPress Blog system as my method of collecting information and posting it to my site (visible only to paying members). With that said, I am trying to automate this process so that I do not have to manually copy and paste the custom form entries into a custom designed blog post. Is there a plugin that can automate this process for me? I will be collecting info such as name, address, phone, email, one photo, and some additional text fields. I want to display them in specific locations within the custom blog post html code…
