Calculation and Ouptut

    I am looking below sequebce of action
    Visitor can enter 4 fields (number) than a formula calculate a output.(calculated field form is suitable upto here)

    Based on the output it will open specifc report/link/video/photo (if and else)

    Also I want to store user input like name and last name somewhere in variable so i can use it in html command in caption=<?php var >

    Any way to accomplish
