    I’m using WordPress 4.5.1, Linux server, PHP 5.6, Wordfence Version 6.1.5

    The Diagnostics Wordfence page reports no issues.

    further down on that page (Wordfence/Diagnostics/Click to view your system’s configuration in a new window) can you see if IPv6 Support is enabled?

    sorry I missed that! Are you behind a reverse proxy (Cloudflare or similar)? Is your own IP reported correctly in Wordfence “Live traffic” if you visit your site from a new browser (not logged in to admin)?
