Looking for a tool/plugin or combination to create a questionnaire that asks you

We are looking for a tool/plugin or combination to create a questionnaire that asks you questions and then gives you a list of your answers in a report along with suggestions based on how you answered the questions. Once you select one of the suggestions you are supposed to get an email with your selection and a link to that tool you selected. It is very complicated. It needs to connect the users email, to the answers they give, and to the selection they make. All of that has to be in a report that emails to them and we can keep track of as well.
