User upload file form?

Hi,

This could quite possibly be a custom job I do myself, separate from WP, but does anyone know of a form plug-in that allows the user to upload a file then send it along with some text in a form?

It’s a big ask, but just in case anyone knows of a solution. If not, I’ll bespoke it ??
