How to get rid of “index.php” from permalinks in IIS

I would like to get rid of the “index.php” from my permalinks. Just about everybody who is running WP on windows has the same wish – but to no avail. I have tried many different methods that I googled. I have yet to find a solution that works.
Any advise would be appreciated. Please note this has nothing to do with the recent bug in 2.6 that breaks your permalinks and can be fixed by the solution provided by 0tt042.
