Incorrect Tag Generation & Thumbnail References

I currently have Windows Azure Storage for WordPress set so that files upload directly to the top level of the CDN.
Basically, file structure is https://cdn.animeherald.com/aniheraldcdn/YYYY/MM/Filename.xyz
And, this worked for quite a while. However, in the past few days, it seems that the plugin has broken in some way. The files are uploading fine, but for some reason, files like the thumbnails and other sizes are being referenced incorrectly in code.
Thumbnails are searching for:
– this directory doesn’t exist on my CDN. Should be https://cdn.animeherald.com/aniheraldcdn/2016/12/Power-Rangers-2017-Goldar-Visual-001-20161219-150×150.jpg
Likewise, actual image references are searching for image references that don’t exist. Example tag below:
Correct files should be:
https://cdn.animeherald.com/aniheraldcdn/2016/12/Miitopia-3DS-Boxart-001-20161219-300×274.jpg
