Issue with DATE

I have ran several tests with this and it is not working.. I have added the date selector option to my form but when I select the date and submit no date shows up on my end…

Here is the form:

[date date-360 placeholder]

[textarea your-message] </label>

[submit “Send”]

AND this is the email I get after I submit the form:
