Suggestion: add fields to the form

Hi,
I customised your plugin to add an extra field to the form, but now I will have to maintain this branch as you update your plugin.
Perhaps this is something you could add to the admin panel, ie the ability to add extra fields to the form. Also allow us to decide which of these are compulsory.
