Simple DB Query Plugin with GUI?

I’ve been scouring the codex today looking for a plugin that replicates this functionality:

User selects item from menu. WP returns data depending on users selection.

For example, the user selects their state, and closest city from a drop down list. The database returns the average home price depending on their location. The actual data would be static and probably manually input by myself.

I can’t seem to find a simple plugin that will accomplish this. Any information would be appreciated.
