plugin elements not working

Hi Guys
Ive been testing this plugin for the past day and a half and Ive installed it on multiple testing sites. but I can not get it to work as it should. some of the issues are as follows.- forms are not appearing in columns even when setting then to 2 or more columns in the setting page.
- Some fields that are being added to a form (registration) are not displayed on profile page or admin profile page. (This makes it impossible to add fields like Address)
- Not being able to add a new users address makes it impossible to use the (paid plugin) that is meant to go with this plugin “Ultimate Member Maps”. I need members be be displayed on a map but the map plugin cant sync with your plugin because it is not saving custom fields (like “Address 1”, “Address 2”, “Town”, “County”) to the users profile.
Can some one please, provide some guidance as to the proper way to set up the plugin (as the Doc are very confusing limited in their information.).
Can send details to testing site if needed. This is a clean WP installation with 2016 theme used, and No other plugin installed.

Where did you increase the max-width?, through settings or with CSS. Is there any information on the other question. I still can seem to get custom fields saved to the user profile page.
