Ninja Forms Conditional Logic does not work with Mega Theme

Hello all,
I have been using Ninja Forms on my website with the theme “Mega” for quite some time. I also bought the conditional logic add-on, because I needed this functionality. Everything went well for some months, until I upgraded WordPress to the 4.5 version, which broke the conditional format function. I thus sent an email to Ninja Forms support about the problem, and they gave me the following answer:
“The reason that this worked when you changed your theme is because it is causing a javascript error on your page which breaks Ninja Forms. In the 4.5 version of WordPress, the javascript libraries were updated. https://www.ads-software.com/news/2016/04/coleman Can you check with your theme developer and see if they have an update to your theme that uses the latest javascript libraries?
I hope this helps. Let me know if you have any questions.”
Fair enough, so I contacted my theme (Mega) developers if they were planning to update the theme to a new version. As an answer I received something extremely vague: “I’m so sorry for keeping you wait about the update information. I have forward you request to my team and they will reply you soon.”
I still have no news on any updates.
As I don’t want to wait any longer, I decided that I want to fix this problem myself. But I don’t really know where to start. As a beginning I checked the Coleman update to 4.5 regarding the javascript libraries that were updated:
These are the libraries that were updated with the coleman update:
jQuery 1.12.3, jQuery Migrate 1.4.0, Backbone 1.2.3, and Underscore 1.8.3 are bundled.
Hereby an example of my website with a test form: https://www.surfawhile.com/testformulier/
Does anyone have an idea of how to fix this issue?
Many thanks in advance,
Hans
