Democracy 2.0.1 – Add a Poll

I have installed the Democracy Poll, can edit the single poll that comes with it, can even cast a vote which is tallied in the database, but when I enter a new question and answers and then click the Submit button to add the new poll, the screen refreshes without adding the new poll.

It’s as though there is some database issue preventing the addition of new table entries,.. or what?

Help in getting this problem solved would be very greatly appreciated.
