multiple radio buttons

I am having one more problem

that i am trying to add 4 items to my radio button.

the field silicon shoe need to have 4 radio buttons(good,bad,ok,improved).

i have already installed contact form plugin.radio button is there but i am struggling to add multiple radio buttons for one field.
