Password Reset Not Sent to Email

Hi-

Have you seen an issue with password reset notification not being sent to email? I have several members per week who enter their username/email to get a new password sent to them, but then the email never arrives to their inbox. I cannot seem to find the source of the problem. Have you seen this occur previously and is there a fix?
