    I wanted to put a shortcode so it would appear on every single post. This I can do. I however wanted to know when the form is used and a visitor submits a message what post was used!! Not sure if this is possible or not.

    You can add a hidden field to your form then set the default value to the merge tag {wp:post_id} This will give you the ID of the post that the form was submitted through.
