Weston, my site also received a similar message, only after the AMP update:

“To owner of https://sanatogapost.com/, Search Console has identified that your site is affected by 1 new Accelerated Mobile Pages related issue. This means that Accelerated Mobile Pages may be negatively affected in Google Search results. We encourage you to fix this issue. New issue found: The tag ‘FORM [method=GET]’ requires including the ‘amp-form’ extension JavaScript.”

@jzlomek The issue is the MailChimp Signup Form. In the upcoming v0.7 of the plugin the inclusion of amp-form will be handled automatically. In the mean time, you can include it manually via code as described in https://www.ads-software.com/support/topic/google-search-console-invalid-amp-tags-amp-form-extension-javascript/#post-9962712
