Love the working of this plugin, but is there any way to avoid displaying any of the personal contact names/emails on the advanced portion of the form?

What part of which file would have to be removed?

I have installed the plugin ’12 Step Meeting List Feedback Enhancement’ but I cannot find how to do the further implementation.
I need to set it up to include the Meeting ID in feedback requests. However, I see no entry box in my WordPress back end to enter this Meeting ID.
Please help!
