The comment title is placed on top of the comment box. Furthermore, changing the position from bottom (B) to the other locations of the CAPTCHA do not even appear in the theme.
Tried to change the CSS of Nucleare but it didn’t help. Does this plugin has its own CSS?

Really not much to say like user before me wrote.
It blocks all spam.
I am just missing one thing. I would like too see all those comments/messages that are blocked.
Is it possible not spam coments are blocked too?

I had to disable this plugin today to check my site for other problems, and in one hour I enabled it again and had over 100 spam messages in that time period.
When I saw this today, I realized how much spam this plugin actually stops. I’m impressed.

I’m using woocommerce and xing template, this plugin does not work in it.
You should be able to make it work in any theme not just wordpress. If it were to be fixed, I will write another review after using it.
