We have activated the plugin on WP 5.4.1 and we are using the classic (non-Gutenberg) editor. When activated we do not get the Extend Link menu button:

I tried this on an image caption, where the caption was also linked. I found that this plugin cannot work with an image caption at all. What it does is duplicates the caption as a text link.
Problem with Images
Extend link will remove the images or duplicate the caption (if captions used). This is because you need the text to link. If you leave the “Text” field box empty, trying to link an image with Extend link, it removes the image and you have nothing there as a result.
Here is what is needed when using Extend link with images in WordPress.
Using with Images – with/without captions
Insert dummy text as “image here” where you want the image.
Highlight the dummy text, use the Extend link.
Go to Text view, hightlight the dummy text, then use the WordPress “add media” button, choose an image and insert.
The dummy text is replaced with the image HTML coding and it is also linked.

]]>For WordPress to ignore this basic kind of functionality is ridiculous.
Normally I don’t even consider low ranking plugins, but it appears this is relatively new to the WordPress plugin depository. So, since I do need this type of added functionality, I installed the plugin.
It is about time this type of plugin is made, it makes dealing with customization of WordPress links so much easier.
I looked at the files for this plugin, nice and small, which contributes to hassel free usage.
I also did a small test, I created a custom link using the plugin, posted it and then deactivated the plugin to see if anything was erased from the customization of the link. Nothing was removed, so that means a person who customized hundreds of links will have no fear of any of his/her work being erased when removing the plugin. – nice ??
