The plugin does not show on my plugin list anymore. When I try to reinstall it, I et message that the plugin is already installed. Which is true since I can find the folder in /plugins. Why is not listed anymore?

Using Formbuilder plugin I am planning to implement an ajax Form submission on a modal popup. So on submission the modal popup shows the success message without page refresh or closing the modal popup, something like an Ajax submission.
Could you please let me know if this can be done via Plugin itself (using its hooks or configurations) or would it need customizations?

I have sets of 5 radio buttons, and the first one indents. I want them all to align (i.e. the first one should not indent).

I hope someone out there answers this, since I’ve seen this problem multiple times in this forum without any answers. Clearly this is a bug.

Hi I’m looking for a plugin which will allow users to register for a
service I provide (Not access to WordPress)
Users would complete a simple form including the following fields:
email address
telephone number
Clicking OK either creates a record or returns a confirmation page informing the user their login details have been sent to them via email.
If creating a record the email address and telephone number are stored and a random unique 5 digit pin. (I could use a mysql trigger to create the pin if the feature is not available)
A new record is then displayed, the pin and user instructions. In addition the details are emailed to the address.
If a record already exists the stored pin and user instructions are resent to the registered email address.
Can this plugin do what I’m looking for? Would be great if it did.

Under the FormBuilder Management (v 1.05) Settings, I have the Default From Address set as (example) [email protected] but when the form confirmation emails are sent out, they are sent from [email protected]. That email address does not exist, and some people are replying to it, getting kickback messages. Can anybody help me figure out why this is happening? Many thanks – Ken Hopson

One of the forms I’ve built using FormBuilder is one that allows someone to subscribe to our email newsletter. We are using Vertical Response to send out our emails. Does FormBuilder have an API I can use to lift the fields from the form (name, email address) and send that information on to Vertical Response’s API handler? If I have to grok the PHP source myself and tease the data out that way, I’m totally cool with that. Just wondering is all…

I’m wanting to track conversions from an email campaign (& later on, from AdWords campaigns). A conversion for this case is submitting a form via formbuilder, and triggering an analytics.js event when the form submits OK.
This would need to be handled by FormBuilder. Could an option be added “javascript fragment after successful submit” as a plugin option?
The example given by Google is:
ga('send', 'event', 'button', 'click', 'nav buttons', 4);
I would enter something like this into the plugin option field, and analytics is already loading in all pages, so the ga function is defined.

We have recently upgraded a website to the latest WordPress version(4.0).
and updated the Formbuilder plugin as well. After the upgrade, a web page which uses a formbuilder form to make payments via secure pay is not working anymore. I did not develop this site therefore I’m not aware of any customizations that might have been done. When I checked the formbuilder form, I noticed it does not show “Securepay” action in the “Action” drop-down.
Also in the error_log I noticed the below PHP Warning.

I’ve seen other posts with similar issues unresvolved – so taking another stab at it.
When I installed this plugin a few years ago, I fully tested it and everything worked fine.
Somewhere along the way, forms stopped being stored and e-mails stopped being sent.
I get no error when I fill out the form and submit it.
What could this be? I upgraded to the latest version of FormBuilder tonight, that did not help.

Just did upgrade to 1.0.1 version of FormBuilder, from plugins section of admin UI.
Upgrade page shows upgrade steps/progress as usual. After last step, clicked on “return to plugins” link.
Action returns blank page (at wp-admin/plugins.php).

]]>Forms that have worked for some time stopped being sent to the emails designated on the day of the upgrade. As a test I created a new form with different email address and it was not sent either. I have talked to my host support and they say PHP and Sendmail are working properly. I do not see any settings that allow me to choose Sendmail, SMTP or PHP.
The forms are created and stored in a SQL database as a backup.
