It would be much appreciated if you could add the ability to format text from certain fields to Title Case such that if a user entered all lower case, or all upper case words their name would be properly formatted and stored in the database or user profile via the GF registration addon. ie: joe blow -> Joe Blow.

I have labels and first label of complex fields off.

I have html5 placeholders on.

These are the 3 settings check boxes I have checked. I created a form with a radio option of phone/email, the field is required.

I then duplicated it. Both forms appear on 1 page and 1 form shows the label for the radio buttons, 1 field does not.

My goal is to keep the label on the radio buttons (which it seems that’s the idea if I follow your code correctly.)

Please advise at your earliest convenience. If you have troubles replicating this issue, I can move a version live (currently it is only on my localhost.)

I know it’s not a problem for end users — so long as errors are not set to appear, but I am receiving this PHP Notice, if you put this up on github I would gladly lend a hand, otherwise I like what you are setting out to do and would love if you could take care of this small thing so admins/developers don’t work around PHP Notices.
