Form creation is easy and straightforward with this plugin. We had the option to integrate with our current form handler but opted to use the form builder instead since it was so simple.

I was searching for a way to build out our raft of Psychology Assessments and put them on our website. Our requirements are complex. For every program we do, we have three assessments that have to be linked together consecutively. They have multiple scales per assessment with complex scoring by scale, by assessment, and number of questions answered. They have to be timed. In addition, one of the assessments requires forced choice ranking, where participants have to choose an equal number of answers from two columns before they can submit. I looked at over 15 assessment/forms plugins in WordPress. They could meet basic needs, but no one could handle the complex ones. And then I discovered the HIPAAtizer WordPress plugin. HIPAAtizer proved to be exactly what I was looking for! Their team looked at our requirements and was able to build the functionality we required within their platform. They even loaded and configured the assessments. I am very impressed and would recommend the HIPAAtizer platform and team. It is easy to use, coded well, and has great support. The fact that I know the data is secure because of their HIPAA certification is an added bonus. For anyone looking to automate GAD-7, AUDIT, PHQ-9, PHQ-15, ORT assessments, HIPAAtizer will definitely work for you.

The full service feature is the way to go. All I did was send HIPAAtizer my client’s intake form. In one step, we got back an online responsive HIPAA compliant intake form with digital and printable form submissions. Very happy we found this plugin!

As developers, we’ve lost doctors and dentists to Athena and Dentrix because HIPAA is such a hassle. I’m hoping others install and rate so Cappers is motivated to keep improving, because this is the real deal. Cheap for the clients and free for me.

• This is NOT another security plugin. The forms are actually hosted on a separate HIPAA compliant cloud server and you include them via shortcode, snippet, or link.
• The drag-and-drop form creator (not a Gravity Forms addon) has all field types.
• For my first form, I used a dental template out-of-the-box with no changes.
• The doctor wanted printable pdfs, so we used the Word templates with tags. Cool feature. (Spacing can be a hassle. And make sure you re-publish after updates so you don’t pull the wrong variables!)
• Plugin walked me through creating my free developer account right from the dashboard without kicking me off the dentist’s site.
