Thanks for this work. It’s a good idea to denormalize some of the most-used meta key / value pairs. (Should the post code data type be a SMALLINT? I don’t think so.)
It’s conventional for a plugin to remove any data it creates upon either deactivation or deletion. I noticed your plugin’s tables were still present after I deleted it (while testing). It would be user-friendly to drop those table from within your plugin.

I activated this plugin and enabled some test CPTs to work with your plugin. I get this error. I am working with Metabox for CPTs, CFs. In this test, I have not enabled any custom tax. I have 1 CPT, 2CFs for the same.

Plus, I am not aware of what the Whitelist and Blacklist are for Custom Fields? I also do not see any custom tables created for the CPTs that I have enabled to work with MO plugin. I can see 4 custom tables were created after plugin activation:
