I am making a membership clinic website in WordPress. Every patient will create her/his unique account. Each patient will have a records sheet (or a facts sheet, if you like) containing data like: full name, date of birth, surgery date, etc.
All this data will be imputed via a form and displayed in a nicely formatted HTML records sheet. The question is: will the MJM Clinic plugin allow me to make records sheet with as many columns as needed? How?

How can I do this ? I can not find the relationship between doctors and service . Please help me out ASAP.

I have a service which can be provided by many specialists (staff member), let’s say a massage. I have three different specialists who provides massages. I set up the info about the three specialists, and assigned to them that service (in the “Additional info” section). But when I display the form in the front end, the form only shows “Service”, “Location” and the other related data, but it doesn’t show any options in order to let the user select which specialist he/she wants to be assisted by.
So, my question is. How could I do that: assign various specialists to the same service and let the user pick one of them.

Is there any way I could add some extra fields to the reservation form?
