how do I add extra form field to post content

I am working on a project and needing extra custom form field eg text input. How do I add it to the post content area? This should be done so that in each individual post. I can place each price amount in each of the category product post item. This should be controlled by the admin only and not the user

Custom Form Fields Disappear in WP

In WordPress > Easy Appointments > Customize…

1) I created 3 custom fields and saved the screen.
2) In preview, I am able to see these fields.
3) In WordPress, once I click off of this Plugin, then click back to Easy Appointments, the custom form fields disappear.
4) I create the custom form fields again. Then preview. And now in preview the form fields show twice.

Cannot access/edit custom form fields in WordPress. How can I edit or delete when they are not visible in WordPress?

Hello and thank you for your great plugins! Please see below for what we’re hoping to achieve.

The plugins we have installed are: Affiliates and Affiliates Contact Form 7 Integration

Our goal:
1. A visitor comes to the site from an affiliate link. The main call-to-action is to have them fill out an online application form (created using Contact Form 7)

2. We’d like the affiliate link to auto-fill a field with the affiliate’s ID, username or login (some kind of identification) so that the visitor doesn’t have to “select” anything or enter a name.

We would then like to map that info to our client’s Velocify CRM system to help track those leads. But, I’ll know more about that once we understand what’s possible for the auto-fill.

Can a Custom Form Field be required "if"

I have a couple of questions that applicants need to answer when registering with the site. If they say “yes” I need then to fill out a comment box. Can I make it so that if they say “yes” then the comment box is required to have content?

Custom Meta Form Field output inbetween posts

Hi, I am trying to output two custom fields from a form in between posts, right after the main post content and before the date meta. I have poured over the codex but I don’t understand it.
I am not new to wordpress, but I am not advanced with php. I am able to manipulate it following directions but can’t write it from scratch.
I am proficient in html/css… and have gotten along just fine with wordrpess by looking through codex and other posts without asking questions on forum until now.
I am designing a site for a client who wants reviews on her site instead of a blog. So that’s what I have set her blog page as, a review page. We’ve disabled “single posts” pages… so the reviews are all output to the listing page (theloop), with the regular meta between posts.
We have the paid version of formidible, a plugin that makes form creation easy. It has an option to make forms that make posts. I set this up and all was great until the custom fields that I added to the form only output on the single posts page, and not on the listings page. It would be perfect if it just would output to the listings page exactly as it does to the single posts page. But we have single posts pages disabled and I want those fields to go between the posts, because it is the reviewers name and city.
So, I poured through the code like I always do and I cannot figure out why these custom fields output to the single posts page (single.php) but not to the (index.php) page. I’m using a child theme for 2012 theme. After examining the code on both index.php, single.php, content.php and functions.php and I think the answer is on content.php or functions.php… I can’t understand why its doing it on the single page but not the listing page can anyone help me? I already tried getting help from formidible but they just told me that it was wordpress not them so they couldn’t help me.
