but and some of problems on the forum were not resolved…so this could help somebody with the same error…
if your site is hosted in an internet provider pls contact your
the technical support of your hosting provider…where your site is
hosted…then asked them to make changes on the mod_security to allow the url on your wordpress folder site ( wp-content ), you dont have to do anything because the hosting provider only have access on it…they usually know what to do.
or if the tech support is not available or if your site is hosted on a hosting provider that does not offer FREE tech support… then check the permission of your “wp-content/uploads” folder..
the “uploads” folder should be set to “777” permission and its subfolders but be careful in doing this on your own, my advise is you should coordinate with your hosting provider to do it for you.
if you are finished uploading the images/pictures the picture files can be set to “644” permissions.
if you guys have other suggestions or violent reactions pls free to contradict…

jQuery seems to be working just fine on my dashboard. However, when I try and upload a media file I get the thickbox popup without any issue but it loads with “jQuery is not defined” errors. As a result, the drag-and-drop box never appears and the “select files” button and “browser uploader” link do not work at all. I can’t insert a media file or set a featured image which is a serious problem.

jQuery seems to be uploaded fine. I’m currently fiddling around with this domain and I see jQuery loads well – https://minecrafthacksz.com/wp-admin/load-scripts.php?load=jquery . Am I experiencing an issue with permissions? I haven’t altered any since fresh factory install but this issue has arisen on my other sites that used to be working fine.
Please help, I really need to update my Blogs – I run around 10 WP blogs that earn me money I rely on. All of my top earning blogs have been giving me this issue after updating to 3.3.1. I am the kind of person who finds my own answers through google or existing threads but I’ve tried everything. I am willing to pay someone to provide a solution – perhaps manually fix it.

My install is located at mydomain.com/cp_wp, so the rewrite rules above look to be correct as far as I can tell. My workflow for creating a post and uploading images for a gallery:
New post
Add title and text to the post
Click insert media-images
Select Files (flash uploader)
Select 5 images, wait for upload/crunching
Click “save all changes”
In the Gallery section below, click Link Thumbnails to Attachment
Order by Name
Click Insert Gallery
Thus far, everything works. The post displays when I visit the site, the gallery works (which a could of minor behaviors that differ from dev that I can’t figure out, but overall it works), etc. However, if I return to the admin site and edit the post, when I click on the gallery section and click “Edit Gallery”, I get the error:
Not Found
The requested URL /cp_wp/wp-admin//media-upload.php was not found on this server.
Apache/1.3.41 Server at www.mydomain.com Port 80
I’m at a total loss because I did not encounter this issue at all on my local development machine during testing, and I have replicated my files and settings exactly. It only occurs out at my ISP.

Anyway – everything was fine until I went to put an image into a post… I can upload just fine, and can see all the images in the Media Gallery -but if I try to insert one, I get the following:

However, when I look at the folder via the FTP, I can see this file is plainly there.
The error message upon attempting to insert media also goes to to say:
“Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request. “
I do believe trying to figure this out is about to give me a seizure! I’ve installed and set up many other WordPress sites, but this is crazy. I also cannot seem to install any sort of auto-plugin or upgrade as it keeps asking me for the FTP login and password – which I know I have correct – yet it won’t let me in. I know it’s correct, because I can clearly get into everything via FTP connection.
I see others having issues with that – which isn’t quite so bad because I can manually install plugins, themes, etc. Sure would be nice to be able to take advantage of the 2.7 upgrade to do this, though.
Thoughts? The Media thing is going to drive me CRAZY!

I have also tried to insert an image that I’ve been using on almost every single post since I set up my WordPress, but I got the same message.
I have checked that the media-upload.php file is in the correct directory, and it is, so I am not sure what the problem is. Would anyone be able to help?
