2 Problems – TIcket no. is same and on-submit Ajax spinner does not stop

I am having a peculiar set of problems, which did not happen during my evaluation. Using the basic version (not paid):

1. All tickets created have the same ticket #.
2. All tickets created and their contents, is showing up in each of the listing (as if it was an exact copy of the earlier (as comments)

3. On click submit ticket button, Ajax is fired, the spinner gif comes up on screen – and nothing else happens. When the page itself is reloaded, it returns with Ticket listing.
