2 text boxes for entry ? Problem and resolution

Hello,

I am new to this message board thing. I was attempting to create a page for my business where there are open tickets that I go on ..
Each ticket has a problem then a resolution.
There might be a plugin for what I am looking for but as of now I just have it set up as a standard post.

I would like to have 2 boxes for entry..
1 for the problem
AND
1 for the resolution

I would also like to have a small area where I could enter a 10 digit ticket number.

and lastly I would like to have a check box that you could check and a list of people that I could check where it would email the resolution to …

I know that there is a lot listed above but I thought that there might be some of what is listed already out there…
