3 Address Fields Showing on Address Fieldset

Hi Matt. Just want to tell you that I really like the plugin so far. I definitely appreciate the user-friendly UI. I’m having one issue though, the Address field (inserted via Address button)is not displaying properly. For some reason, it shows up w/ 3 Address Fields:
1)Address
2)Address
3)Address 2I made sure that my theme isn’t causing this by changing to Twenty Eleven. I has to be something in the php or jQuery code. Here’s the link to the form:
