3 little issues with More Fields

Hello I need help with 3 issues in WP More Fields.
1. I’m trying to use this plugin to be making a database that serves as a record of my workshop services lubricants for cars. I wish I knew how to put a small branded logo instead of only the name at the top.
2. Also see if you can change the size of text fields (also selecion list) and be able to put next to each other.
3. With the date option can not get the output format is dd / mm / YYYY.
When therefore selected date is normally the calendar but when I choose the date changes the output format to YYYY-mm-dd
I hope you can help me with these little problems.
