4.0.0 upload doesn’t replace

Working within WP Engine we are attempting to replace a PDF. Uploading the replacement document doesn’t replace the file and even sometimes reverts back to the previously uploaded version. Almost like it is not playing nice with caching. Had to revert back to 3.6.3 to get it to work reliably.
