404 error after changing image alt text in editor

Weird problem that I never seen before and I have been developing with WordPress for awhile.

Using a custom post type and pagination I am getting a 404 error after selecting another page number. Now I now how to fix this but I have tracked the issue to editing alt tags of images through the editor. If I reset the permalinks the pagination works until a alt or title tag for a image is enter and saved. The image can be anywhere on the site as long as it is done through an editor and Bam! The pagination breaks again and requires for permalinks to be reset again.

I am lost ion the sauce on this one. Maybe someone has ran into this before.
Any idea’s..

I have already went through all the plugins and basic steps to getting debugging something like this.
