    I upgraded one of my sites to 3.1 and thought all was working fine, then I found 404 errors.

    I’ve reset the .htaccess file by deleting and letting WP create a new version. I’ve tried disabling all plugins and still … 404 from Pages. I even tried deleting and creating the pages again, but the same error happens.

    Any suggestions would be greatly appreciated.

    does it reappear if you temporarily switch to default permalinks?
    if on a windows server – you might need to read this thread

    That helped … some.

    Here is what I did:
    1) disable Advanced Permalinks
    2) select default permalinks
    3) test: home page, post, categories, tags, and pages work.
    -old permalink structure gives 404 (/%category%/%postname%/)
    4) select permalinks “Month and name”.
    5) test: home page, post, categories, tags, and pages work.
    -old permalink structure gives 404 (/%category%/%postname%/)
    6) activate Advanced Permalinks
    -pages fail to load
    -home page, posts, categories, tags all still work.
    -old permalink structure loads

    I disabled Advanced Permalinks, for now. My backlinks won’t work.

    I have the exact same issue described by Troy P. It occurs with no plugins activated on a 3.1 Multisite Network with domain mapping.

    As Troy mentioned, the Advanced permalink structure works for posts, but not for single pages.
