404 page randomly disappearing

On July 18, I added the 404 plugin. Worked great. Using the Focus theme.
In the last week or so, I recently upgraded to WP 3.6, Genesis 2.0.
I now am seeing the 404 page randomly disappearing after the first iteration of 404.
Especially when using IE. I am on IE10.
When I type in the website url and then for testing put a random page after the /, the page not found works the first time typically. However, when I put another random set of characters, I get the default IE 404 message.
Both FF and Chrome seem to be work okay with the plugin. It’s only IE where starts defaulting to their 404 page.
