Yes, you can create such form. To split the form into pages you just need to add a “Page Break” type field on a form where you want “breakpoints” to be. All the fields above the “Page Break” field will be on one page and all those below it will be on next page(s). You can use multiple “Page Break” fields on one form so in order to split the form to 5 steps you would need four of them in total.
You can also use conditional visibility rules for fields and that will be carried over to next pages. You cannot set given “Page break” (page) to be conditionally visible but you can set fields that way.
Once you got form “split” into pages you’d just want to put two “sets” of fields on your 3rd page and set their visibility (for each field accordingly) to depend on value of your field from 2nd page.
Then when user fills-in the form, they go through page one, reach page two and make some selection(s), move over to page three and on that page only relevant fields show up (depending on a selection made on page 2).
