a custom template for a page I get a error

I am trying to install this template on the video page of my blog and I get this error message. Does anyone know what it could be? Thanks in advance..

P.S the entire theme has already been upload through FTP. All I need is the layout of the theme which I have been trying to use the index.php of that theme.
